How do I report a side effect to the MHRA?

The Yellow Card scheme is the UK's official way to report suspected side effects or problems with medications. It is run by the Medicines and Healthcare products Regulatory Agency (MHRA).

What you can report

  • Side effects, even mild ones
  • Suspected adverse reactions
  • Faulty or defective medicines
  • Suspected counterfeit medicines
  • Quality concerns

How to report

Anyone can report — you do not need to be a healthcare professional.

  • Online: yellowcard.mhra.gov.uk
  • By phone: Yellow Card Scheme helpline
  • By the MHRA Yellow Card app: available on the App Store and Google Play (this is the MHRA's own app, separate from Bliss)
  • By post: Forms are available from the MHRA website

What to include

  • The medication name
  • The batch number if you have it
  • What happened, when, and how long it lasted
  • Any other medications you were taking at the time

Should I tell Bliss as well?

Yes — please contact our team. We may submit our own Yellow Card report for serious or unexpected events.
